In an effort to accommodate as many of our patients as possible, we offer extended office hours during the work week and also on the weekends. We feel that access to your primary care physician is one of the most important aspects of good healthcare. ![]() The secure pre-check-in link is sent to you via text or email and works on various devices, including smartphones and tablets. Our Patient Pre-Check-In process offers a way for our patients to complete parts of the check-in from a personal device before arriving at the office, minimizing time spent at the front desk. Minors will need to check in with their parents or guardian at the front desk. Our self-check-in kiosks are located in suite 101 (our main suite) on the first floor to expedite your check-in experience. We are not accepting new adult patients currently. We do not accept Workers Compensation Insurance AND we are currently only accepting new patients who are infants and adolescent patients and young adult patients transitioning from a pediatrician’s office. Our care team will work towards ensuring that you receive the appropriate age-related preventative testing and treatment as well as the latest evidence-based treatment (proven and documented) for your individual medical condition(s). They will be in charge of the care team of nurses, Nurse Practitioners, Physician Assistants and Diabetic Educators who might interact with you when you visit the office. They will maintain an ongoing relationship with you and oversee your care. ![]() The physician you choose will be your Primary Care Physician. As part of this care model, we ask all our patients to select one personal physician. One of our main goals at CNY Family Care, LLP is to provide all our patients with continuous and comprehensive health care. We have a Diabetes Care Management Team, that is recognized by the National Committee for Quality Assurance, and the American Diabetes Association for Excellence in Diabetes care and self-management. They work directly with our physicians to provide all of our patients with the best family care possible. Our Nurse Practitioners and Physician Assistants are individually board certified and licensed by the State of New York. All of our physicians are Board Certified by the American Board of Family Medicine, Academy of Family Physicians.
